bff forms to Complete & Return with registration form:
(For all campers attending a weekend program unless otherwise indicated)
Health History Form (For Campers with Diabetes)
Health History Form (For Campers without Diabetes)
Developmental History (All Campers)
CGM and Cell Phone Policy (for campers with T1D who will be using a CGM at camp)
HIPPA Form (All Campers)
bff forms due at least 1-week BEFORE camp session starts:
Intake Form (for campers with type 1 diabetes - must be completed within 3 weeks of the start of the program)
Copy of School Physical completed by camper’s doctor (for campers without diabetes)
Administration of Medications at Camp Nejeda (All Campers - for medications besides/in addition to insulin)
